Manual lymphatic drainage (MLD) is a very gentle, specialised massage technique that encourages excess fluid to move out of swollen tissue. After [liposuction in Dubai](/plastic-surgery/lipohp) it is one of the most commonly recommended parts of recovery, typically starting within the first one to two weeks once your surgeon confirms it is safe to begin.
It is also one of the most misunderstood. MLD is supportive care that helps many patients feel and look better sooner; it is not a miracle treatment, and the wrong kind of massage at the wrong time can do genuine harm. Here is the honest version.
## What manual lymphatic drainage actually is
MLD is a light, rhythmic technique that follows the direction of the body’s lymphatic pathways, using strokes so gentle that first-time patients are often surprised anything is happening at all. It is a specialised skill with its own training, and it has nothing in common with deep-tissue, sports or “recovery” massage. If someone is kneading, pressing hard or working into muscle, whatever you are receiving, it is not MLD.
## Why it is recommended after liposuction
Liposuction removes fat through small tunnels in the tissue, and in doing so it temporarily disrupts some of the fine lymphatic channels that normally clear fluid from the area. Fluid therefore accumulates, which is a large part of why swelling after liposuction lasts as long as it does. MLD encourages that fluid towards lymphatic pathways that are working, which can reduce tightness, improve comfort and help swelling settle. It works alongside, never instead of, the two foundations of swelling control: your compression garment and daily walking.
## What the evidence honestly supports
Honesty matters here, because MLD is heavily marketed in Dubai. In many patients it genuinely improves comfort and helps swelling settle, and it is widely recommended by body-contouring surgeons, including within the Brazilian tradition Dr. Valente trained in. What it is not is a guarantee. It is supportive care: it does not guarantee you will avoid fibrosis or lumpiness, and your healing biology, the surgical technique and consistent garment wear matter at least as much as any massage. Anyone selling MLD as the thing that “makes or breaks” your result is overselling it.
## When it typically starts
Most surgeons have drainage begin somewhere within the first one to two weeks after surgery, once incisions are settling and the tissue is ready for even gentle work; starting aggressively in the first days helps nothing. The right start date varies with the areas treated, the amount of swelling and how you are healing, so your individual plan is confirmed at your follow-up appointment rather than fixed in advance. The same applies to [how many drainage sessions you need](/blog/lymphatic-drainage-sessions): typical courses run somewhere between five and ten sessions, adjusted to your swelling rather than sold as a package.
## Choosing a therapist in Dubai
Dubai has excellent post-operative therapists and, frankly, some poor ones. Look for a therapist with recognised MLD training and real experience with post-surgical patients specifically, and be wary of anyone offering deep, vigorous or painful “post-lipo massage”. Aggressive pressure on healing tissue can worsen swelling and bruising, cause significant pain and disturb tissue that is still settling into its new contour. A useful rule: proper MLD should never hurt. If a session is painful, stop and speak to your surgical team.
If you are planning liposuction, or you have had surgery and are unsure what your recovery should include, raise drainage at your consultation or follow-up. Dr. Valente will give you an honest view of what MLD can and cannot do in your case, and a plan matched to your healing rather than to a spa menu.
