Nasal Reconstruction
Nasal reconstruction is surgery that rebuilds a nose damaged by trauma, tumour removal, congenital malformation or an inadequate previous operation. It restores the skin, cartilage, bone and internal lining of the nose partially or totally using local or distant flaps and grafts.

What is nasal reconstruction and who needs it?
Nasal reconstruction restores deformities caused by any type of injury to the nose tumour removal, trauma and can also correct congenital malformations (for example, deformities associated with cleft lip or hemangiomas) or inadequate results from previous surgery. It differs from cosmetic rhinoplasty: rhinoplasty reshapes an intact nose, while reconstruction rebuilds missing or destroyed structure. The two overlap in technique but not in starting point.
Patients typically arrive with one of a few stories: a skin cancer excision (including Mohs surgery) that removed part of the nose, an accident that collapsed the bridge, a rhinoplasty that went wrong, or a deformity present since birth. Breathing problems caused by structural damage are an equally valid reason to seek assessment.
How nasal reconstruction is performed
The nose is rebuilt layer by layer reconstruction covers four things: the skin cover, the cartilage framework, the bone, and the internal lining of the nose. Which technique is safest for your case is explained at consultation, prioritising your expectations alongside what your anatomy allows.
- Local flaps nearby nasal or cheek skin moved to cover small defects, matching colour and texture closely
- Forehead flap the workhorse for larger defects; forehead skin, which matches nasal skin best, is transferred in stages
- Skin grafts thin coverage for selected superficial defects
- Cartilage grafts taken from the septum, ear or rib to rebuild support so the nose neither collapses nor blocks breathing
- Composite and multi-stage reconstruction complex defects are rebuilt across several planned operations
Multi-stage surgery is the plan, not a complication
Surgery takes between 1 and 4 hours depending on complexity, and more complex reconstructions such as forehead flap work involve separate operations some weeks apart. This is a normal, planned part of the method. Nasal reconstruction can be surgically demanding, and a full understanding of the limits of the outcome is fundamental: you should be prepared for the possibility of multiple planned surgeries and psychologically ready for this phase. Practical care that supports healing: sleep with the head elevated on your back, avoid smoking completely (it directly impairs flap survival), protect the nose from sun while scars mature, avoid glasses resting on the bridge until cleared, and return to exercise progressively on clearance.
Scars, breathing and the result
Will there be visible scars? Surgery can leave scars, but incisions are positioned to hide in the natural folds of the nose or inside the nostrils wherever possible. Forehead flap reconstruction leaves a forehead scar that typically heals well within natural lines. Scars mature and fade over 12 to 18 months, and scar revision is possible later where needed. Honesty matters here: reconstruction trades a defect for planned, positioned scars.
Can I breathe normally afterwards? The goal is to restore both the appearance and the function of the nose, and in most cases normal nasal function can be achieved though this varies with the extent of the original damage. Rebuilding the internal lining and cartilage support is what keeps the airway open; a reconstruction that looks right but blocks breathing is a failed reconstruction.
